Referrals

Thank you for entrusting us with your patient or client’s care. We will make contact with your patient in 3-4 business days. Once we have completed our evaluation with your patient, we will send you a copy of the report.

Please notes that faxes and records requests are received directly by Triangle Neuropsychology Services, though the fax header may display "NeuroBridge Assessments." Both names refer to the same practice and provider team — no action is needed on your end.

To submit a referral, fax pertinent records to 919-384-9683 and include patient name, DOB, contact information, insurance information (if applicable), and reason for referral.
