‍ Please read these Terms of Service ("Terms") carefully before using neurobridgeassessments.com (the "Website"), operated by NeuroBridge Assessments® PLLC, a brand of Triangle Neuropsychology Services, PLLC, a professional limited liability company formed in North Carolina ("NeuroBridge," "TNS," "we," "us," or "our"). This Terms of Service contains important information regarding limitations of our liability. Your access to and use of this Website is conditional upon your acceptance of and compliance with these Terms. These Terms apply to all visitors, users, and others who access or use the Website. By accessing or using the Website, you agree to be bound by these Terms. If you disagree with any part of these Terms, you do not have permission to access or use the Website.

About NeuroBridge

‍NeuroBridge Assessments is a brand operated by Triangle Neuropsychology Services, PLLC ("TNS"). Clinical evaluations, scheduling coordination, insurance verification, billing, and clinical recordkeeping for NeuroBridge clients are all handled directly by TNS. This Website is used for informational purposes and to initiate a scheduling or contact request; it is not used to conduct evaluations, process payments, or store clinical records.‍

Our Services‍ ‍

NeuroBridge provides fully remote (telehealth) neuropsychological evaluation services, including cognitive health evaluations, adult ADHD testing, academic/professional testing accommodations, and second-opinion/documentation reviews. Services are conducted by licensed neuropsychologists via telehealth, available across most U.S. states participating in PSYPACT.

‍Pricing for evaluations depends on the type of assessment, insurance status, and individual clinical needs. Fees are provided directly by our staff prior to scheduling and may vary based on the scope of evaluation, insurance coverage, and referral source. Telehealth Services‍ ‍

Evaluation services are conducted remotely by telehealth using a secure, HIPAA-compliant videoconferencing platform. Your Consent for Telehealth Consultation, along with the specific technology used for your consultation and testing appointment(s), will be reviewed and provided separately as part of your clinical intake with TNS. Telehealth is not accessed through this Website. By requesting an evaluation, you acknowledge that:

  • Services will be provided via telehealth rather than in person, and telehealth carries inherent limitations compared to an in-person evaluation (e.g., reliance on your equipment, internet connection, and self-reported environment);

  • Telehealth is not an emergency service. In the event of an emergency, you must call 911 or go to your nearest emergency room rather than attempting to reach your provider through a telehealth platform;

  • The videoconferencing platform(s) used provide videoconferencing technology only and do not themselves provide medical or healthcare services, advice, or emergency/urgent care;

  • You are responsible for ensuring you have access to a private, distraction-free location and a reliable internet connection for your appointment(s);

  • You will not share your telehealth or testing appointment link with anyone not authorized to attend your appointment, in order to protect the confidentiality of the session;

  • Availability of services may depend on your physical location at the time of the appointment, consistent with PSYPACT and applicable state licensing requirements, and it is your responsibility to accurately confirm your location for each session.

‍A full Consent for Telehealth Consultation, including the specific risks, benefits, and terms of the technology used, will be reviewed and signed separately as part of your clinical intake with TNS, and that signed consent controls over any conflicting statement in these Terms.

Insurance and Billing

‍NeuroBridge currently accepts Medicare, BCBS, and Aetna. All insurance verification, claims submission, billing, and payment collection is handled directly by TNS through its own systems — not through this Website.

‍We are committed to assisting patients in understanding their insurance benefits. As a courtesy, our team will make every reasonable effort to verify your benefits and provide guidance regarding deductibles, copayments, coinsurance, and potential out-of-network coverage. Our practice can assist but cannot guarantee payment or coverage.

However, insurance benefits are ultimately the responsibility of the policyholder. Because you purchased and own your insurance policy, you are financially responsible for knowing and understanding your plan's deductible, copay, and coinsurance amounts; whether our practice is in-network or out-of-network for your plan; any prior authorization, referral, or pre-certification requirements; and any changes to your coverage, effective dates, or plan limitations. We strongly recommend that all patients contact their insurance carrier directly to confirm coverage for the specific services they are seeking from our practice before scheduling.

‍Some insurance plans list us as out-of-network but still offer out-of-network (OON) benefits that may partially reimburse you for services. If your plan includes OON benefits, we can help you submit claims or documentation when appropriate; however, the availability and amount of any reimbursement is determined solely by your insurance company.

You are responsible for all charges not paid by your insurance plan, including any unpaid balances, deductibles, copays, or non-covered services. Insurance can be billed only when there is a medical need for the evaluation; if the reason for an evaluation is entirely for legal purposes, insurance cannot be billed and self-pay rates apply.

Cancellation, Rescheduling, and No-Show Policy

‍We ask that you provide as much advance notice as possible if you need to cancel or reschedule a telehealth appointment, as evaluation appointments require significant advance preparation and hold time that could otherwise be offered to another patient.

Fees. A fee of $150.00 will be charged for any appointment cancelled within 3 business days of the scheduled appointment, or for any missed appointment. For same-day testing appointments, this fee is $400.00. This policy applies consistently and without discrimination to all patients.

How Fees Are Applied. Fees are charged 5 business days after a missed or late-cancelled appointment if a waiver has not been requested. To request a waiver, submit a verbal or written request to our billing office within 5 business days of the appointment, along with any supporting documentation. Our office manager will review all requests and respond promptly.

Emergency Exceptions. A waiver may be granted for unplanned emergencies that could not have been reasonably anticipated at the time of booking, including: ‍

  • Unplanned hospitalization of the patient or an immediate family member (spouse/partner, child, parent, or sibling)

  • A positive test for a contagious illness (e.g., COVID-19, influenza, strep) within 48 hours of the appointment

  • Death of an immediate family member

  • A government-declared weather or public safety emergency

‍ Documentation may be requested to support a waiver review.

One Exception Per Calendar Year. Each patient is eligible for one (1) emergency waiver per calendar year, resetting January 1st. If a second qualifying situation arises within the same calendar year, the standard fee applies in full.

Clinical Nature of Services

‍Our services are clinical in nature, and the goal of our evaluations is to aid in diagnosis and clinical decision-making. While a written report may be useful in other contexts (e.g., academic accommodations, workplace accommodations, disability determinations), NeuroBridge makes no guarantee that a report will produce any particular outcome in those contexts. Our clinicians provide an accurate and objective assessment based on testing and clinical judgment, regardless of the referral source's or client's expectations.

‍We do not guarantee a specific timeframe for delivery of results or written reports. While most clients receive reports within about a week of testing completion, complex cases may take longer. The written report reflects the results of testing, clinical interview, and background information/records provided to us. Factual errors may be addressed through an addendum or correction process, but the clinical content, interpretations, and conclusions of a report are solely at the discretion of the evaluating clinician.

Text Message Consent and Disclosure

‍If you provide us with your mobile phone number, we, or TNS on our behalf, may use it to send appointment reminders or other service-related communications by text message, where you have opted in to receive such messages. Message and data rates may apply, and message frequency may vary. You may opt out at any time by replying STOP, or by contacting our office directly. We do not share your phone number with third parties for marketing or promotional purposes.

Remedies

‍To the fullest extent permitted by law, and except with respect to clinical services provided to you as a patient (which remain governed by applicable healthcare and professional liability law, not this section), you agree that your sole and exclusive remedy for any breach of these Terms as it relates to a purchase or scheduling request made through this Website shall be to pursue the dispute resolution process described in the "Governing Law and Venue" section below.

‍You agree to use this Website in accordance with all applicable laws, rules, and regulations, and these Terms. You agree not to:

These Terms are governed by the laws of the State of North Carolina, without regard to its conflict of law principles, regardless of your state or country of residence or origin.

Any dispute arising out of or relating to your use of this Website or these Terms — other than disputes related to the actual clinical care and treatment provided to a patient, which are governed separately by applicable healthcare, informed consent, and professional liability law — shall be brought exclusively in the state or federal courts located in Durham County, North Carolina, and you consent to the personal jurisdiction of those courts.

If any provision of these Terms is found to be invalid or unenforceable by a court of competent jurisdiction, that provision will be enforced to the maximum extent permissible, and the remaining provisions of these Terms will remain in full force and effect.

Changes to These Terms

We reserve the right to modify these Terms at any time. Changes are effective upon posting to this page. Your continued use of the Website after changes are posted constitutes acceptance of the revised Terms.
